We use Octopus.  Push the Database MSI out to the Database Role and deploy it, and deploy the Application MSI to the Application Role.

Our first application deployed with Octopus just went into production, but I've been using it since 2007 when I was at Microsoft.

Installation on 2 nodes, but both going to just one database.  We want to insure that the database only gets installed on the DB server.
